Here’s how to make a referral

It’s easy, a referral can be made by your social worker, your community nurse and by yourself if you are the main carer. Whoever wishes to make the referral needs to speak with Wayne on 0121 308 6555. They will take a few initial details over the phone and send out to you a referral pack; this pack contains information about Skills Tank and a referral form for you to fully complete.

Once complete, return the form to us and we can then gain a better understanding of your needs and the needs of the adult you care for. A good way to progress from here is to book a FREE Skills Tank taster day, this will give you the chance to get a good feel for Skills Tank meet the staff and sample a typical day and for the adult you care for to come along and enjoy a day with us.

As part of the day we can have a more in depth discussion based around the details contained in the referral form – let me assure you at this point that every bit of information we hold is treated with the utmost respect, it’s kept securely and is covered by the data protection act. Our discussions will form part of an assessment process which helps us to determine the level of service we need to give you.

Our assessment process and the details you provide to us helps us to determine;

  • The personal care needs that are needed
  • The medical background to the adult you care for and what we need to put in place
  • Personal interest, both leisure and social
  • What behaviours are possible and in what situations
  • The religious beliefs we need to be aware of
  • The communication needs of the adult you care for
  • Any health and safety considerations we need to make

This isn’t a long-winded exhaustive process but a very important one as it helps us to work with you closer and whichever professional services are involved so that the best level of support and provision can be determined. It’s important to us to deliver a provision that will stimulate and progress an individual.

With your funding coming from the council, the NHS or a mixture of the two you will need to be referred to Skills Tank via a social worker and/or community nurse. If you don’t have a social worker, we may be able to direct you to the correct place to start the social worker allocation process off. If you already have a social worker for the adult you care for you will need to discuss with them the options available to you other than Skills Tank so that you have a clear choice.

Please note: If you are receipt of direct payments, personal budgets, independent budgets or wish to fund your Skills Tank provision from private funds you can refer directly to us – see above for details.
